Reporting a Claim

If you need to report a personal insurance or business insurance claim, you have several options. Use whichever option is most convenient for you. Be sure to report your claim in a timely manner.

Direct to Us

  • Stop by our office at 1767 Northampton Street, Holyoke, during our normal business hours (Mon-Fri, 8:30 AM — 4:30 PM) and pick up and/or fill out any claims forms that are needed.
  • Call your claim in to our office 413-534-7355. We will mail or fax you any claims forms you may need to complete.
  • Return the completed forms by mail to us at Goss & McLain Agency, Inc.,1767 Northampton Street, Holyoke, MA 01041.
  • Or fax them to us at 413-536-9286.


E-Mail Claim Alert

You can also notify us via email that you have a claim to report. We encourage you to use e-mail to tell us about the incident while the details are fresh in your mind. We will get back to you the next business day to review the necessary claim steps. However, in the event of an emergency please call our office any time 24 hours a day, 7 days a week.

NOTE: You will still need to formally report your claim and get the necessary claims forms at our office.
