If you need to report a personal insurance or business insurance claim, you have several options. Use whichever option is most convenient for you. Be sure to report your claim in a timely manner.

1. Stop by our office at 474 Appleton Street, Holyoke, during our normal business hours (Mon-Fri, 8:30 AM — 4:30 PM) and pick up and/or fill out any claims forms that are needed.
2. Call your claim in to our office 413-534-7355. We will mail or fax you any claims forms you may need to complete.
3. Return the completed forms by mail to us at Goss & McLain Agency, Inc., 474 Appleton Street, Holyoke, MA 01040.
4. Or fax them to us at 413-536-9286.
